Has anybody found an easy way to convert Fidelity's historial CSV files
to QIF format?

The best out there is John Woodward's shareware program QIF Master:

Also reads QIF and OFX/QFX files for pre-processing before importing to
Quicken. QIF Master is MacOS X only.
